- Client – Sign up for JotForms Silver Plan via https://www.jotform.com/pricing/ When signing up, sign up via an email account.
- Client – Follow the instructions found at https://www.jotform.com/help/500-how-to-upgrade-to-hipaa-compliance/ to upgrade your account to HIPAA compliance. The last step for “Form Migration” is not required as there should not be any forms in your new account.
- Client – Send us your login credentials to Jotform via secure message: https://secure.coursevector.com
- Client – Send us any forms you would like us to create on your site.
- CourseVector – Create forms, style forms, set up email notifications, embed forms on site, create pages on site to embed forms on.
- Client – Review/request changes/approve forms.
- Client – Change Jotform password so CourseVector no longer has access (needed for HIPAA compliance).
Note: If additional work on Jotform is needed in the future, we will have you send your current Jotform password. After the work is completed, we will require you to change it to maintain HIPAA compliancy.
